Safety Recommendation A-22-010

Recommendation

TO THE FEDERAL AVIATION ADMINISTRATION: Require Textron Aviation to include a checklist item on the Cessna 560XL pretakeoff checklist for pilots to verify that the airplane’s parking brake is fully released before takeoff is initiated.

Safety Issue and Context

The National Transportation Safety Board (NTSB) is providing the following information to urge the Federal Aviation Administration (FAA) to take action on the safety recommendations in this report. We identified transportation safety issues during one ongoing investigation and one previous investigation involving Cessna 560XL airplanes in which parking brake pressure was not fully released before attempted takeoffs, which prevented the airplanes from rotating for takeoff. Once the airplanes reached this point in the takeoff sequence, they were beyond the point at which they could be stopped safely, leading to fatal or serious injuries.

The NTSB is issuing three safety recommendations to the FAA.

On May 30, 2025, Acting FAA Administrator Christopher J. Rocheleau informed us that the agency is continuing the work described in its previous letter regarding Safety Recommendation A-22-10. We look forward to receiving your next update.

The FAA is awaiting the release of Textron's updates to the aircraft flight manuals (AFMs).

May 30, 2025 · Recipient Response · FAA

From Christopher J. Rocheleau, Acting Administrator, Federal Aviation Administration: The Federal Aviation Administration (FAA) continues to collaborate with Textron to monitor the efficacy of the changes made to aircraft documents currently available to operators in response to the issues identified in this safety recommendation. Textron Aviation has revised and published sixteen aviation flight manual (AFM) documents with a temporary change. The publication dates of all 16 AFM documents are as follows: • Three AFM documents (Model series 560XL) were published on February 23, 2023; • Four AFM documents (Model series 560) were published on April 27, 2023;

• Four AFM documents (Model series 550 Bravo and Citation II) were published on May 25, 2023; and • Five AFM documents (Model series S550/551/501/500) were published on August 17, 2023. Additionally, on October 30, 2024, the pre-takeoff checklist for the Ascend version of the CE-560XL airplane with the same type rating was developed but will not be published until a Type Certificate (TC) is issued. Additionally, all related non-FAA-approved checklists are being updated to align with Textron Aviation’s company standard procedures. Textron has not announced when these permanent checklists will be made available for FAA review.

The FAA will continue to monitor the progress as Textron moves towards incorporating the temporary revisions into the upcoming permanent revision of the AFM’s. I will keep the Board informed of the FAA’s progress on Safety Recommendation A-22-10 and anticipate providing an update by May 31, 2026.

May 3, 2024 · NTSB Correspondence · FAA

We note that Textron Aviation is voluntarily updating its airplane flight manuals and pilot checklists for all Cessna 560XL airplanes listed on Type Certificate Data Sheet A22CE to ensure they include consistent text about releasing the parking brake, if set, prior to takeoff. Although you did not require Textron to update these documents, we believe its planned actions represent an acceptable alternate solution to this recommendation. Pending our review of the updated Cessna 560XL pretakeoff checklist, Safety Recommendation A-22-10 is classified OPEN-- ACCEPTABLE ALTERNATE RESPONSE.

August 10, 2023 · Recipient Response · FAA

-From Polly Trottenberg, Acting Administrator: As mentioned in our previous response, the Federal Aviation Administration (FAA) collaborated with Textron Aviation to assess the Cessna 560XL and identify an appropriate action plan in response to the issues identified in this safety recommendation. As a result, Textron Aviation has proposed to voluntarily update all airplane flight manuals and pilot checklists related to the Cessna 560XL listed on Type Certificate Data Sheet A22CE. This update will allow for textual harmonization across variants in relation to releasing the parking brake, if set, prior to takeoff.

This proposed change will apply to airplane serial numbers 560-5001 through 6000 and 560-6001 and impact over one hundred pertinent documents. The FAA will continue to keep the Board updated on its progress and provide records of revision and the revised Cessna 560XL checklist upon completion.

I will keep the Board informed of the FAA’s progress on Safety Recommendation A-22-10 and anticipate providing an update by May 31, 2024.

December 19, 2022 · NTSB Correspondence · FAA

We note that you intend to work with Textron Aviation to assess the safety issues related to the Cessna 560XL. We believe this is a necessary first step to responding to these recommendations. Pending our receipt and review of additional information regarding how you plan to address the specific actions described in each of these recommendations, Safety Recommendations A-22-8 through -10 are classified OPEN--ACCEPTABLE RESPONSE.

July 21, 2022 · Recipient Response · FAA

-From Billy Nolen, Acting Administrator: The Federal Aviation Administration (FAA) is analyzing these safety recommendations to develop an appropriate action plan, which will include working with Textron Aviation to assess the Cessna 560XL in response to the issues identified in these safety recommendations.

I will keep the Board informed of the FAA’s progress on Safety Recommendations A-22-8 through -10, and anticipate providing an update by August 31, 2023.

This letter provides information about the National Transportation Safety Board’s (NTSB) May 4, 2022, report Require Safeguards to Prevent Cessna 560XL Takeoff with Parking Brake Engaged, AIR-22-06. The details of this accident investigation and the resulting safety recommendations may be found in the attached report, which can also be accessed at http://www.ntsb.gov.

As a result of this investigation, we identified the following safety issues:

• Without a parking brake indication, some Cessna 560XL pilots may not recognize that the parking brake is not fully released and attempt to take off, which could result in a runway overrun. • A checklist item before takeoff is initiated would aid pilots of Cessna 560XL airplanes in ensuring the full release of the parking brake before the takeoff roll and prevent hazardous runway overruns. Accordingly, the NTSB makes the following safety recommendations to the Federal Aviation Administration:

Issue an airworthiness directive for in-service Cessna 560XL airplanes to require that they meet the parking brake indication requirements of Amendment 25-107 of Title 14 Code of Federal Regulations
